A. Renewal of Passport

Original passport with photocopies of the 1st page, last page, and page bearing valid employment pass / visit pass / dependent pass

Note: Applicants are advised to come in decent attire; photo will be taken at the Embassy's Consular Section.

B. Replacement of Lost Passport

Police report (from station with jurisdiction over area where loss was reported)

Statutory Declaration from the Magistrate Court or Affidavit of Loss (B$59.50 or US$20.00) from the Philippine Embassy

Special pass from the Immigration (Enforcement Section)

Photocopy of lost passport

B$153.00 for replacement of lost 32-pages or machine readable passport

B$255.00 for replacement of lost e-passport

(cost incorporates penalty, prescribed by regulations of the Department of Foreign Affairs)

C. Replacement of Damaged Passport

Statutory Declaration from the Magistrate Court or Affidavit from the Philippine Embassy (B$59.50) explaining how the passport was damaged

D. Amendment of Passport (due to marriage only)

Original and photocopy of duly authenticated marriage contract (authenticated by the Authentication Division of the Office of Consular Affairs, DFA)

E. New Passport for Minors

Original and 4 photo copies of the Birth Certificate issued by the Registrar of Birth, Deaths and Marriages

Original and 4 photo copies of the duly authenticated marriage contract of parents, if married

Photocopies of the passports of each parent - 1st page, last page and the page bearing valid employment visas.

4 copies of Report of Birth ( Forms available at the Embassy)

B$59.50 for the Report of Birth and B$102.00 for e-passport (44 pages)

Personal appearance of either parent together with the child. If filed by a third party, parent's Special Power of Attorney should be presented.

F. Marriage

Certification from the Office of the Civil Registrar General, National Statistics Office, that he or she has no record of marriage, authenticated by the Authentication Division of the Office of Consular Affairs, DFA

Written consent to marry by father/ mother/ surviving parent or guardian, or persons having legal charge, for contracting parties between the ages of 18 and 21 years old.

Statutory Declaration of No Legal Impediment to Contract Marriage - B$59.50 each for bride and groom (if both are Filipino nationals intending to marry in the Embassy)

Legal Capacity to Contract Marriage - B$59.50 (for Filipino Nationals marrying foreigners)

G. Special Power of Attorney

Passport of the applicant

H. Affidavit of Support and Consent

Passport of the person executing the affidavit

I. Authentication of Documents

Original documents to be authenticated and one certified true copy by the magistrate court of Brunei Darussalam.

Original and photocopy of receipt issued by the Land Transportation Office, Philippines
